1 PT -needed in Kingman, AZ -
Client Name: Select through Banner
Setting: OP Ortho - Adults
Start date: ASAP
Guaranteed Weekly Hours: 38-40
Length of Assignment: 13+ weeks
Experience Requested: Open to NG w/ OP rotation - prefer 1+yr OP exp
Therapists On Staff: -Other PT's on staff
Documentation System Used: Therapy Source/Net health
Patient Caseload: 10pts/8hr day on average. -Contingent -on arrival rate and complexity of patients.
Work Schedule/Patient Scheduling: -M-F -
Bill Rate: $68-75 DOE BR - no% (may be able to get higher since its more remote) -

Any overtime must be pre-approved by management.
Expectation is that documentation is to be done at point of service for all travelers & perm employees.
If census is low or PT has cancelations, expectation is for provider to ask what they can do to make sure they hit their guaranteed hours for the day/week. Examples of things we have had providers do are: helping the front desk, calling patients, filling paperwork, cleaning up, etc. If they do not want to help out in order to hit their GTE only actual hours will be billed/paid.
4 week notice required for any days off not listed in ADL.
30 day notice for client/provider -
If -clinic is closed/closes early or has delayed start due to inclement weather only actual hours will be billed.
**Up to 8hrs unbillable orientation for 1st day. PT will need to document on their timesheet how many hours spent on training before seeing 1st patient. Can possibly get this waived for providers that have exp w. the EMR &/or have worked at another Select facility.
